OK, I’m not re-inventing the wheel here. Thankfully – I’d probably make it square or something…

This is a fairly standard contact form using Flash and PHP . So what’s interesting about it? OK, it has a few interesting features :

- first off, it looks quite cool :)
- nice, à la mode reflections
- frighteningly easy drag n drop installation
- it’s all contained in one movie clip and one php file
- full validation of all fields if required (just uncomment some code)
- error bullets and error message appear when fields not filled in
- check for correct email address (@ and .)
- space-efficient : labels are built into text fields, and disappear on click
- tab through fields with your keyboard
- nice spinner animation while waiting for confirmation from server
- thank you page on successful send
- sorry page on failure

So there we have it. A nice, clean form, easy to install, looks nice and does the job well. Enjoy!

hi there, like this file allot. just wanted to ask if this file also sends a ‘confirmation’ email with a pre-writen message in it? would be mega useful if it could.

No, I am afraid it doesn’t do that. It just shows an onscreen message which at the moment is “your message was successfully sent”, but which you could customise to be anything you wanted.

Hello I have purchased your contact form and have tried to use it on several servers pointing back to my yahoo account and I always receive an error sending message. Not sure what is wrong wondering if you could assist me in getting this working. I am using servers that support php and the accounts are hosted thru Godaddy.com. Your prompt response would be greatly appreciated.

Not sure what the problem is. It depends on the config of your servers. I understand that PHP5 causes problems with my mailer.php file. If however you are using PHP4 , then it could be something to do with the way you’ve integrated the form into your Flash site?

Nice form!

Can the code be changed to work with Coldfusion instead of PHP ? (I’m not expecting you to do it, I just need to know if it is just a simple process of the form posting to some page, the page sends the email and sends some variable back to flash). If that’s the case, I’m ready to buy!

Hi,

How does one change “surname” to something else. I’m a novice with Flash; but if the change is done via Flash, under which layer would I make the change. I couldn’t locate that text using Flash 8 when I went through the layers.

You have to change it in two places : in the Flash file, and in the “mailer.php” file.

In the Flash file, dbl-click the movieclip “mailerclip” to edit it, then immediately doubleclick the movieclip “myForm” to edit that, and you will see a layer called “as”. All the instances of surname are there, in the code.

In the FLA , when you see it in quotes, written in green, it’s the string which appears in the textfield, and when you see it without quotes, written in blue, it’s the actual name of the variable itself.

In the php file it’s the variable you are changing, not the string.

I don’t know whether you wish to change the variable itself, or just the displayed string, so it’s up to you to do as you see fit. Cheers!

I’m new to PHP . I’ve uploaded this file to my server which supports PHP but after clicking send the “wait” just spins forever. Any advice?

Your form looks great but its not working, I am using godaddy as a host with php4. Can you help or is it a waste of money?

The form is compatible with php4 but has some issues with php5 (the mailer.php file needs rewriting for php5).

I am surprised it doesn’t work – if Godaddy really uses php4 it should work fine. Has Godaddy blocked the php function “mail()”? Some shared hosting providers do this. Check their documentation?

If so, I’m afraid yes it is a waste of money, as would be any other form that processes mail with the mail() command :-(

Hi riczito, Great contact form I like it heaps. However I am having trouble getting it to show in Dreamweaver when I preview in IE. Like it shows up fine within Dreamweaver but not in IE?? What am I doing wrong? thanks, Dave

when i receive the emails this is the way it looks:

this is the messageName : mark Tel : 555-555-5555 E-mail : dfgf@dda.com

Message :this is the message

For some reason the message is displaying before the Name.

I would rather it just to start off with the name. Any Help.
