Thanks for the feedback. I hadn’t really thought of checkbox/radio/dropdowns but now that you mention it, it seems like it would be pretty useful to have for fields such as the “type of website” one. I will definitely incorporate that into a v2 if there is any.

I’ll add that in a v2, but what are you looking for? The standard Adobe component radio buttons and drop downs, or custom ones made for the form, but not as easy to customize?

Very easy to use and setup. I like how fast the the form works after submitting. Only thing missing is the checkbox/radio/dropdowns.
