What if the “Message fails” – there’s no way to get back or close the form…

Yes… but that can be fixed by adding a few frames so the close button can be visible in all the movieClip…
If you have problems with that I’ll help you out 
Not working for me. Form appeared for a brief second then vanished leaving just the green background.

You get “message failed” because of flashden services…
if you have this files on a server with PHP support it will work without any problems
@apearce – PHP
@gueromixx – just add some more frames with that close buttons… when you are in the button timeline hit F5 until the frames are over the whole scene
